Community Wellness & Resource Hub is a new nonprofit clinic in Monroe, Louisiana, providing primary care and community navigation services. We are ready to serve more patients, but we do not yet have a consistent source of referrals. Residents who could benefit from our services may not know we are here, and organizations serving them may not know when or how to refer someone to us.

We have participated in community outreach and started building relationships with schools, community organizations, and service providers. These efforts have opened conversations, but we have struggled to turn those connections into ongoing referral relationships.

Our main barriers are limited staff time, uncertainty about which partnerships to prioritize, and the absence of a clear outreach and follow-up process. We need support developing a focused referral partnership strategy that our small team can realistically implement.

Addressing this challenge would help us connect more Monroe residents with care, use our limited resources effectively, and support the clinic’s long-term sustainability.

The pro bono consultant will create a written referral partnership strategy for Community Wellness & Resource Hub that includes:

• A prioritized list of referral partner types, such as schools, community organizations, and service providers, identifying which partnerships our small team should pursue first and why.

• Key messaging explaining the value of our primary care and community navigation services, whom we serve, and when and how partners should refer someone to us.

• A simple follow-up process our team can use to turn initial conversations into active partnerships and consistent referrals, including suggested follow-up timing and a basic partnership tracking template.

Success will mean our team has a practical, ready-to-use strategy that clearly identifies whom to approach, what to communicate, and how to follow up. This will help us build consistent referral relationships and connect more Monroe residents with care.
